Colorado residents: order your free loss-history report
To request a free copy of your A-PLUS loss-history report, you must first complete a Request for Disclosure Form.

Three easy ways to order your loss-history report:

1. FAX:
   Download and complete the PDF Request for Disclosure Form, print a copy, and fax to: (201) 469-4140.
2. Mail:
   Download and complete the PDF Request for Disclosure Form, print a copy, and mail to:

   A-PLUS Consumer Inquiry Center
   545 Washington Boulevard, LOC 22-6
   Jersey City, NJ 07310-1686

3. Telephone:
   Call the A-PLUS Consumer Report Request Line at 1-800-709-8842, option 2. Leave your name and address, and we will mail you a Request for Disclosure Form. If you prefer we fax the form to you, please leave a dedicated fax number. When you have completed the form, mail or fax it to A-PLUS as directed above.
