Electronic Affidavit Submissions for Countrywide and Texas (CSP+ and TCLSP)

Each company/group reporting statistical data to ISO is required to submit the applicable affidavit(s) attesting to the completeness and accuracy of your submitted data for the applicable account year. The company official's name entered on the form constitutes an electronic signature.

Instructions:
To gain access to the affidavits, you will need to provide your 4-digit GIRD number, (without the alpha at the end) and 4-digit ISO statistical reporting company number.  Once you have logged in, you can choose which affidavit form you need to complete.

If you are completing on behalf of a group:

• For the Texas Affidavits, you will need the NAIC number for each company in your group. 
• For the States other than Texas Affidavit, you will need the ISO company number for each company in your group.

A company/group is also required to submit an affidavit if they have submitted NIL Transmittals (c/w), Zero Submission Control Records (CSP+), or Zero Transmittal Records (TCLSP). 

Please note that you cannot save a completed copy of the affidavits to your computer.  If you would like a copy for your records, please fill in required information and then print it before submitting.  When you are finished, click the submit button.  If you need to complete another affidavit, click the back button and choose the next form.  Once you have completed, printed, and submitted your required affidavits, click the sign out button to exit.
